James’s Journey to Freedom:. Overcoming the OCD Monster with Victoria Lumley's Guidance

“Before, I was afraid. I was scared. I was anxious. I felt isolated. I felt confined. I was not me. It wasn’t normal. My OCD was severe, and the OCD Monster took over my life. I felt controlled. I couldn’t do what I wanted to do. It was like a war between me and the monster. I felt like I had no help at all. Doing everything 3 times, constantly washing my hands, completing everything perfectly until it was PERFECT. But no one is perfect. So my compulsions had no logical outcome. It was a trap. I thought I had lost all hope… until Victoria and CBT!

When I first met Victoria via video call, I felt nervous, but also determined to defeat this monster who completely controlled who I was. On the first session we had together, we completed a check-up… to see how severe my OCD was. And it was much worse than I thought. But the only way to defeat a monster, is to trick the monster. Victoria’s ways of overtaking the OCD monster were genius. We began portraying the monster on paper, imagining what it looked like and what it was feeling and why it was forcing my brain to do things I didn’t want to do. We started testing compulsions, making the monster think that I was going to give in… but that was the trick. At first, I did give in, and that was completely fine, because it was a learning process. As the sessions went on, I progressed each time, and eventually, I won the battle.

Gradually, I became VICTORIous more and more. The OCD monster was shrinking day by day. I finally felt in control of my life again; even if it was just a small part of my life, it was still a small part of my life I had control of. And that was another battle won.

 Over time, I became more triumphant. It was like a war. I was conquering the OCD monster until it became a piece of NOTHING! I remember the few days where I felt completely in control of my life again. It was just so… freeing. I was excited! I was elated! I was over the moon! I felt FREE! I WAS FREE!

 All of mine and Victoria’s hard work had paid off!

 Don’t get me wrong, I sometimes have little slips, little bumps in the road where the monster comes back for a split second, and then disappears again. Those times will always occur. But that is just who we are.

 Victoria has not only helped me with my OCD, but also with any problems I have in my life. She is such an AMAZING person who I will always remember. I had a very positive experience of online therapy. I always felt there was an overall plan and that we were working towards a goal together which is something that has been missing with previous therapists where I felt almost responsible for directing therapy or that I was just coasting along. I feel I connected with my therapist very well and again, compared to other therapists, I felt she was able to challenge me and appreciate where I was coming from. Each session seemed to achieve a lot although she was very flexible and I quickly became aware that I could raise anything that had come up that week or something that had been affecting me and was happy to work on that rather than what might have been planned. We did some work on updating an aversive memory and looking back I think that we did this at a pivotal time when I had built up a level of understanding, knowledge and insight that allowed me to really get the most out of re-scripting the memory. Therapy was delivered in a way that felt relaxed but again always had direction. My therapist used visual aids which I found really useful and she would often email me with a summary of a session - I referred back to these emails countless times when I was struggling. I found my therapist to be very professional and knowledgeable but also kind and empathic. 

Did you have any reservations about using online therapy? How quickly were these overcome?

I had slight reservations about online therapy but soon overcame these. I was a little bit nervous before the first call but my therapist put me at ease straight away and I felt that by the end of the first session I was not at all worried by the format. I sometimes struggled with looking at myself on screen but mostly I found that being in my own setting and the distance between my therapist and I actually allowed me to relax, say more and bare all.

What aspects of online therapy appealed to you or helped you choose this over traditional office based therapy? 

There was no access to specialised therapy for body dismorphic disorder (BDD) in Scotland or at least nearby and I felt I needed someone experienced in this area so my options were therefore limited. Online therapy enabled me to access a specialist from the comfort of my home. I have attended therapists before and did find sometimes it was a struggle to go to the office so I suppose in retrospect online therapy from home appealed because it took the pressure off.

Would you recommend online therapy to a friend? Please explain your answer.

I would certainly recommend online therapy to a friend. As someone who has been in therapy before I think that I found it easy to adapt but others may take a little longer. I would certainly recommend it in terms of accessibility and I don't think I cancelled one session with my therapist during our months of therapy whereas in the past I have felt that depression or anxiety have been factors in me not making the journey to a therapist's office. In the comfort of my own home I felt therapy was more accessible. I didn't feel at all intimidated whereas at office-based therapy I have often felt under the microscope.

Was there anything that you felt stopped you getting therapy in the past?

I have tried a few therapists in the past but my online therapist Lisa is the first who I felt was experienced with the issues I was encountering and was also an intellectual match. I would say in my case it was access to someone with specialist knowledge in BDD that was the main aspect that was difficult for me. I became frustrated with the therapy I was getting that seemed to skirt around what I had identified as what I felt was a huge issue that was impacting my life. I even had one therapist who agreed with me that I was perhaps suffering with BDD but then didn't seem to know how to help me with it at all.

What helped you to seek therapy this time?

Having a diagnosis of BDD from Professor David Veale and being signposted to someone that was accessible to me and who had specialist knowledge was mainly what encouraged me to give it another go.

How has accessing online therapy impacted your life? What are you able to do now they you were not able to do before?

My life has improved greatly since having accessed therapy with Lisa online. Even just having a diagnosis and a deeper understanding of how my behaviours were reinforcing my beliefs was huge for me but to be able to actually break these negative cycles with my therapist's help has allowed me to engage more with the world around me. I am able to notice or call out negative thought patterns and this has resulted in a huge shift in my beliefs about how I look and I am also able to see myself as more than just my appearance. I have mechanisms in place to help me enjoy big events such as weddings or parties where before these would cause me months of anxiety and I would suffer for weeks after. I am able to enjoy my love of fashion more - prior to therapy I wanted to wear clothes that didn't make me stand out and would often throw out expensive clothes I had bought after one wear that I felt I looked ridiculous in. I have been able to actively engage with online dating. I am now able to frequently walk down the street with my face exposed to the world and not concealed by a scarf. I no longer spend long periods looking in the mirror or take videos of myself and reflect on them endlessly. I do have relapses and I do not always feel great about how I look but I do feel that I have tools to bring this back within normal parameters. I know that members of my family have noticed that I seem more open and my negative behaviours and defences have reduced massively and I feel that this projects back on me.

Please give some feedback about your experience of therapy?

Fantastic! My therapist's methods were the most effective and professional I have experienced. She clearly is focused on results and the patient (as opposed to maximising revenue by slow rolling treatment). Over the course of ten sessions she quickly gauged my comfort levels, applied an appropriate amount of pressure to push me to complete CBT tasks and challenge OCD, and taught me the basics of the theory and practice so I am empowered to be my own therapist.

She occasionally provided therapy for longer than the scheduled hour if we were in deep discussion. This is very generous and further highlights her priority is the patient’s success.

What aspects of online therapy appealed to you or helped you choose this over traditional office based therapy? 

Convenient! Can do from home at night to fit around busy schedules.

Would you recommend online therapy to a friend? 

Definitely. I can see why some people might have a reluctance to pursue online therapy over traditional therapy, but I found it better than in-person therapy I’ve had. The quality comes from the therapist, not the delivery method (e.g. Skype vs in-person).

I also think it’s maybe easier to focus on the message in therapy via online sessions in the comfort of one’s own home. In person, on someone else’s office, additional social dynamics can come into play, even subconsciously, that can distract and potentially detract from the therapy itself.

Was there anything that you felt stopped you getting therapy in the past?

It was hard to make it work around schedule and hard to find and access the ‘right’ therapist. Also was probably in denial that I needed help.

What helped you to seek therapy this time?

My therapist came recommended from Professor Veale and was available for appointment times I could make, in my location (online was perfect for my circumstances).

How has accessing online therapy impacted your life? What are you able to do now they you were not able to do before?

Significant impact. The therapy enabled me to change unhelpful behaviours that had persisted for years, greatly reducing my levels of OCD. The changes made have held fairly strong since therapy ended. Crucially, I now have a much greater understanding of my own thought processes and beliefs and how to better navigate and manage them. Home life is much improved, I’m less tired, and things generally feel a bit easier.

Advice to others who might be considering seeking online therapy

I was quite surprised that CBT wasn’t a bit like I thought it would be. Like the pragmatic approach, that not focusing on the past but looking to future. 

I had massive reservations, I'd never had any therapy before. I felt maybe there was a stigma about it. After a few sessions I realised it was going to be quite useful, it took a lot the emotion out of it and made me realise I have the power to change things rather than thinking that the way I am is set in stone. 

I liked the convenience of being able to do the therapy in the evening around the baby, and from my own home so no need to organise a babysitter. 

I was dubious that online therapy would give you the one to one personal experience. But it works. Its good for busy people with out the flexibility to spend time coming into an office. You can have your session in the comfort of your own home. Minimal stress or fuss to lead you on a path to a better you. 

I always thought it was for people with ‘real' problems and that my issues weren’t that important or relevant and that I should struggle through because I wasn’t depressed or ill or anything. CBT gives you useful life skills to help make your life work the way you want it to. And I hadn’t realised that. I thought to go to therapy was to admit weakness rather than realising it shows strength to communicate. 

I have learnt to be less hard on myself and to give myself a break, no one is perfect. To look at the world in a different light, and to move towards making better choices in how I behave towards my friends and family.

I felt relaxed and comfortable as I was in my home surroundings and what was most convenient for me was fitting in the sessions around work. It proved to be a very convenient way of accessing a therapist which was specialised my my phobia and anxiety disorder without the need for travel. 

Talking with Lisa to me was like speaking to a friend or confidante. Lisa made me feel very relaxed and made the sessions informal yet offered very informative constructive advise on phobias and exposure therapy. What I specifically liked about Lisa's therapy is that it was proactive and homework was given - you got to test things out. It was almost like setting up little experiments and you could actually test out situations and exposure work in comfortable surroundings.  I really liked the fact that it was not just talking about the past and reflecting - it was more based on changing behaviours in the present and looking at how this changes the way we think and feel going forward. 

It is very convenient and this worked for me in many ways - with both work and my social life. I would have had to visit London to get the therapy I needed for my specific phobia and Lisa helped me access the much needed specialised treatment without the cost of travel or the inconvenience of having time off from work.

Did you have any reservations about using online therapy? How quickly were these overcome?

Skype sessions actually took quite a lot of pressure off me because I didn't have to worry about getting to and from the location and being in an unfamiliar environment which in the beginning would have been a major issue for me. I felt comfortable and at ease at home and could relax. It was a bit weird at first talking to someone you had never met in person, but i knew after the first session I already felt a small release in the grip of my anxiety and so this was overcome very quickly. 

Thinking back to before your first session, is there anything you would have liked to have known about online therapy therapy before you started?
I don't think so. I'd been through regular face to face therapy before so knew how the process worked. There really wasn't much difference to be honest, in fact, I felt more comfortable over Skype and it was much easier to fit in to my routine. 

Do you have any specific comments / feedback about me as your therapist? About my style, session content and delivery of online therapy?

I immediately trusted Lisa because I immediately felt better. I never felt like I was being pushed to take steps I really wasn't ready to take, instead I felt positively challenged to prove to myself I was capable of managing in situations I was worried about.

Would you recommend online therapy to a friend?
Yes, definitely. It's difficult when you have a full-time job to fit therapy in and if you suffer from an anxiety disorder then therapy is essential. Skype based, online therapy is brilliant as you can do it from the safety and comfort of your own home, fit it into your schedule and relax. Something I realised was being anxious all the time is absolutely exhausting. It would have been hard for me to get through an entire day of work and then travel again to get to therapy and then home again. This made my life and my recovery as easy as possible and I actually enjoyed it more than face to face sessions.

(OCD - 12 Sessions)

It hit us like a truck, we did not see it coming, we are informed parents and always hoped that we would recognise any mental health problems or issues with substance abuse etc. P has always talked to us and did on this occasion - but the words he said made no sense at all - what on earth was happening? 

His condition felt very unstable and unsafe at that time. The person we knew was barely visible and appeared to be constantly diminishing. We could not rationalise with him about his thoughts and each time it felt as though we may be making some small headway we realised it had been a false hope? The easiest thing would have been to leave him in bed each morning, keep him at our sides and protect him until we got the help required. He was not functioning normally at all and needed prompting and guidance for the most basic of daily activities of living and self-care?

We did not know if we should be forging ahead like parent steam-rollers forever pushing, coaxing, pulling, encouraging, were we adding to the problems? ...but frankly the alternative did not seem to offer hope either so always 'doing something' seemed preferable to 'doing nothing' and watching the decimation of character unfold, unchallenged?

A combination of anti-depressant medication, and reading and taking tips as we read and reread through Dr Veale's book whilst waiting for a consultation moved P from the edge of the precipice by the time he spoke to Prof. He had become much more rational and less afraid by the time of his assessment - things felt safer and steadier. If his school had not been so supportive we would not have been as well-placed as we were either... so yet again we were fortunate. 

We were in no doubt however that we had secured only very temporary 'holding' measures, and could see and indeed experienced, the huge tendency for the OCD to overwhelm. I cannot begin to imagine how individuals who do not not have the support and resources manage this illness, which impacts so heavily on every aspect of life for them and their families.

To have the recommendation from Prof Veale for CBT with you was like receiving a golden key. We knew and know that in effect you provide the recipe but P has to follow it carefully.

He has his ingredients and has had to learn how to best utilise them. 

All that we read had pointed us to seeing his time with someone with your skill base as being crucial if he was to secure any chance of recovery. We believed that time was of the essence, the longer the OCD controlled behaviour, the harder it was going to be to resist and change. From his first session with you it was apparent to us all that finally P may have a chance of tackling this illness head-on, he returned from it with a quiet confidence and a little self-belief and was subtly more upbeat about everything.

The effect on the household as everyone begins to exhale is positive and so it is like a synergy - your hour impacts so massively - as we were able to pull-back our support gradually (because you provided P with the skills) this in turn enhanced his self-belief and his ability to achieve once again and so on. Each week another step up and reinforcement of what had gone before and importantly tactics to help deal with the inevitable blips. Your feedback from sessions also helped us to better understand where we could best support or indeed pull back our input and were invaluable - thank you. 

There needs to be many more ‘Lisa people’ out there.

 

Jess's Experience of Online Therapy for Emetophobia 

To be perfectly honest I was the first one to be skeptical about online therapies after going through more than a decade of traditional face to face therapies. However after realising that my condition needed the attention of a specialised professional I decided to try it. Travelling was not an option for me at the time so the online approach was the only viable alternative. 

I was practically sure it wouldn't have worked but I couldn't have been more wrong. I was afraid the lack of the physical presence would have made the therapy somehow cold and distant making it impossible to relax, open up and therefore inevitably being an unsuccessful therapy. I quickly understood the warmth and the comfort of the therapy depends on the therapists and the feeling you have with them. I do believe not all therapists work for everybody. There must be a certain affinity, a good match. I was lucky enough that I clicked instantly with my therapist even tough she was across the continent. She made me feel comfortable, she was engaged at all times, she always faced the camera which quickly made me forget there was a camera in the first place.

My therapist has not only been incredibly present and involved during the sessions but has always followed up with summaries and helpful tips to get me through the tasks which were set during the sessions.

I went from being the biggest skeptic to the biggest believer that online therapy is the most incredible gift for people who cannot find suitable help in their own vicinity. 

Doing online therapy allows you to access it from the comfort of your own home, which to many Emetophobes could be an incredible help considering they might be very uncomfortable going out or travelling long distances.  Knowing that there is help available, as easy as a click away, is of incredible value and if you struggle with this difficult life companion and you haven't yet reached out for help I would strongly advise you to get some help from trained professionals as this is a very specific condition and therefore it requires a specific approach and treatment.

 

Leanne’s received specialist CBT treatment for OCD through My Therapist Online and reclaimed her life again.

I live in wales and access to getting good quality CBT/EPT for OCD is very limited.

Whilst searching online I came across ‘My therapist online’ and emailed to see if they could help me. They carefully looked at all their therapists experiences to allow me to work with a therapist who was best suited to the help I needed.

Previously I have had CBT for my OCD but have never felt I have recovered to a point where it doesn’t interfere with my daily life. I feel very comfortable during our Skype session ( I often wear my pyjamas as I have evening appointments) and feel each session is focussed and moves me along in my recovery. I have developed a self- help therapy file and the homework that I undertake always focussed me very closely on building a resilience to the ‘OCD bully’.

This time last year I thought I was stuck and my recovery stage was as good as it could get. I thought I was going to have to deal with this quality of life ( restricted by OCD) forever. Now I am focussed in work ( have a stressful and demanding job), enjoying married life and travelling to many parts of the world.

Grainne has taught me many techniques and showed me, how  exposing yourself to fears reduces anxiety and has made me realise the world is not such a scary place.

I am now excited about life, my future and the things I have to look forward to.

I know I will always have OCD and that I will need to manage it, I am also sure I will have times in life when my OCD rears it’s ugly head again. But with Grainne help and my new confidence in dealing with OCD I feel I can deal with it and cope with any setbacks.
